Feedback, Complaints and Disputes

If the participant wishes to give the Provider feedback, the Participant can talk to Kate Caldow on 0474499793 or in writing via email to admin@cctherapy.com.au

If the participant is not happy with the provision of supports and wishes to make a complaint, the Participant should submit the details of the complaint in writing to Lauren Clancy, Business Manager via email at accounts@cctherapy.com.au.

All complaints will be followed up directly with the Participant.

If the Participant is not satisfied or does not want to talk to the above people, the Participant can contact the National Disability Insurance Scheme Quality and Safeguard Commission by calling 1800 035 544, or visiting one of their offices in person. More information can be found by visiting www.ndiscommission.gov.au/about/complaints

Complaints can also be made to the Australian Health Practitioner Regulation Agency via their website or by phoning 1300 419 495.
